Position Summary
Smoky Mountain Home Health and Hospice is seeking a motivated and relationship-driven Community Educator (CE) to join our Morristown office. This role is responsible for developing and maintaining strong referral relationships within the community while educating healthcare providers, facilities, and organizations about our home health and hospice services.
The ideal candidate will have a healthcare-related background , strong communication skills, and a passion for connecting patients and families with high-quality care.
Key Responsibilities
· Build and maintain professional relationships with physicians, hospitals, skilled nursing facilities, and community partners.
· Educate referral sources on home health and hospice services, eligibility criteria, and benefits of care.
· Identify opportunities to grow referrals and expand community presence.
· Conduct in-person visits, presentations, and outreach activities within the service area.
· Collaborate with clinical teams to ensure appropriate patient referrals and smooth transitions of care.
· Track and report outreach activities, referral trends, and market feedback.
· Represent the organization professionally in the community and at networking events.
· Maintain knowledge of services, programs, and regulatory guidelines.
Qualifications
· Healthcare background required (examples include CAN, LPN, RN, Medical Assistant, COTA, or other clinical/health-related experience).
· Previous experience in healthcare marketing, outreach, and business development.
· Strong understanding of home health and hospice services.
· Excellent interpersonal, communication, and presentation skills
· Highly self-motivated, proactive, and results-driven with a “go-getter” mindset.
· Demonstrated ability to independently identify opportunities, initiate outreach in new settings, and build relationships from the ground-up.
· Strong follow-through skills with the ability to maintain and grow relationships over time.
· Ability to build and maintain professional relationships.
· Strong organizational skills with attention to detail.
· Valid driver’s license and proof of current auto insurance.
Schedule
· Full-time, Monday-Friday (8:30 AM- 5:00 PM)
· Schedule may include community events, outreach activities, and weekends as needed.

Additional Information
This is a field- based role serving the Morristown and surrounding areas. Please note that Smoky Mountain Home Health and Hospice does not offer relocation reimbursement for this position. Due to the travel/on-call requirements associated with providing care within our service area (Hamblen County), preference may be given to applicants who currently reside within or near the community served by this role.
In addition, specific compensation (pay) details are not discussed prior to a formal employment offer.
